Personal Information (in Clinic)

During your consultation we will request additional personal information, via a patient consent form. This is needed in order for us to provide the best service possible to you. We respect your trust in us, and take seriously our obligation to safeguard your personal information.  

We will make notes regarding the consultation, against your file, in order to provide you with the best possible service and treatment upon follow-up.

You have the right to request a copy of any of the personal information we hold about you, submitted via the website or at the clinic, and to ask for it to be corrected if it is wrong.
